Position SummaryThe Residential Supports Coordinator is responsible for providing comprehensive support, case management, and crisis intervention services to youth and young adults across Ozone House's residential continuum. This position works with clients who are experiencing homelessness, at risk of homelessness, and residing in Ozone House residential programs to promote stability, well‑being, and increased self‑sufficiency through a client‑centered, trauma‑informed approach. The Residential Supports Coordinator conducts thorough assessments, develops individualized service plans, connects clients with internal and external resources, provides advocacy, and monitors progress toward achieving safe and stable housing outcomes.
This role combines direct residential support with case management functions, ensuring that youth receive holistic services that address their immediate needs while building long‑term independent living skills.
- Bachelor’s degree in social work or equivalent combination of training & experience
- 2+ years of experience working with homeless, runaway, or other youth in crisis
- Experience facilitating groups and knowledge of group dynamics
- Comfort with outreach work and meeting clients in their own homes or in the community
- Ability to effectively engage youth with diverse identities, including intersecting identities of race, ethnicity, socioeconomic background and sexual orientation/gender identity and expression
- Ability to effectively manage crisis situations
- Ability to collaborate effectively with public systems and other providers
- Commitment to best practices including Positive Youth Development, Harm‑Reduction, Housing First and Trauma Informed approaches
- Work requires effective written and verbal communication and interpersonal skills; ability to produce quality work within tight time frames and simultaneously manage several projects; ability to participate in and facilitate group meetings
- Possess strong work ethic and ability to take initiative. Demonstrate a high level of personal and professional integrity and trustworthiness
- Successful completion of pre‑employment and annual background check; valid MI driver’s license, current insurance, negative TB test, and access to reliable transportation
- Provide case management services to homeless and at‑risk youth and their families, including resource information, linkage, coordination, advocacy, and other assistance
- Maintain an active caseload of 15 – 20 clients at any given time
- Conduct initial and on‑going assessments of strengths and barriers to healthy functioning. Assess needs, make eligibility determinations, and facilitate referral or placement of clients seeking emergency, transitional, or independent housing assistance
- Conduct comprehensive assessments for clients that will help in identifying strengths and challenges and will assist in the development of a service plan
- Provide independent living skills training to clients, including budgeting, nutrition, home care, leases, personal hygiene, healthy relationships, computer literacy and online safety, employment and education skills and support, and civic engagement
- Utilize intake and assessment information to develop an individualized service plan in conjunction with the youth that identifies goals, action steps, services needed, and time frames with an outcome primary outcome of safe and stable housing
- Assist and advocate for clients in obtaining important documents for youth that may include, but is not limited to state ’s, driver’s licenses, birth certificate and social security card
- Assist clients in locating, obtaining, and maintaining safe and stable housing
- Assist and advocate for clients in accessing and utilizing education and employment services and support.
