What You ll Do In This Role:
The Social Worker plays a vital role in empowering older adults to live safely and independently in the community. As a key member of the Interdisciplinary Team, this position provides compassionate, participant-centered case management, counseling, advocacy, and resource coordination that enhances quality of life and supports each participant s unique goals. Working with a high level of autonomy and clinical expertise under the guidance of the Social Work Manager, the Social Worker builds meaningful relationships with participants and families while connecting them to the services and support they need to thrive.
