The Licensed Vocational Nurse (LVN) provides direct nursing care and supports the delivery of clinical services to residents of Casa Serena Congregate Living, Inc. The LVN practices under the supervision of a Registered Nurse (RN) and in accordance with physician orders, resident care plans, Title 22 California Code of Regulations, California Department of Public Health (CDPH) requirements, and facility policies and procedures.
Full‑time position: NOC Shift (3x12 hr shifts). Includes 1 rotating weekend day every other week.
Responsibilities- Provide direct nursing care within LVN scope of practice.
- Administer medications and treatments as ordered and per facility policy.
- Monitor and document resident condition and response to treatments.
- Collect specimens and perform basic clinical procedures as authorized.
- Assist with admissions, discharges, and transfers as assigned.
- Supervise and support CNAs in the delivery of resident care.
- Communicate changes in resident condition promptly to the RN and physician.
Support nutritional care by ensuring residents receive meals, nourishment, and snacks consistent with physician‑ordered and RD‑approved diets.
- Reinforce dietary restrictions, texture modifications, and fluid restrictions as applicable.
- Monitor and document intake/output as assigned.
- Support resident participation in activities outlined in the facility’s weekly activities plan.
- Encourage engagement consistent with resident abilities, interests, and care plan goals.
- Protect resident rights, dignity, privacy, and confidentiality.
- Comply with abuse prevention and mandatory reporting requirements.
- Maintain a safe environment and report incidents, injuries, or hazards promptly.
- Participate in emergency preparedness and response activities.
- Document nursing care accurately, timely, and objectively per facility policy.
- Ensure CNA documentation is reviewed and addressed as required.
- Participate in required facility in‑services, mandatory trainings, and competency validations.
- Ensure documentation and practice reflect compliance with Title 22 requirements.
- Current, active California Licensed Vocational Nurse (LVN) license, in good standing.
- Background clearance as required by law.
- Knowledge of CLHF regulations and nursing standards.
- Ability to supervise and support unlicensed staff.
