Position: Older Adults Nurse

Job overview

We are seeking a compassionate and experienced Older Adults Nurse to join our team at New Coningsby Surgery
. This is an exciting opportunity to make a real difference by delivering high-quality, person-centred care to residents in local care homes.

Main duties of the job

About the Role

As our Older Adults Nurse, you will play a vital role in supporting older adults in the community.

Key responsibilities include:

  • Care Coordination: Acting as a vital link between care homes and the GP practice. Attending weekly neighbourhood team meetings and carrying out weekly visits to each care home.

  • Patient Assessment & Management: Undertaking comprehensive health assessments for adults aged 65+, supporting chronic condition management, and working proactively to prevent avoidable hospital admissions.

  • Non-Medical Prescribing (if qualified): Prescribing medications and treatments within agreed clinical protocols.

  • Wound Care: Providing specialist care for chronic and complex wounds, including Doppler and ABPI assessments.

  • Immunisations: Administering vaccinations in line with national schedules.

  • Collaboration: Working closely with GPs, care home staff, multidisciplinary teams, and families to ensure holistic care.

  • Additional Duties: Including suture/clip removal, ECGs, venepuncture, and pathology requests where required.

Person Specification Qualifications Essential
  • Qualified Nurse
  • Registered with NMC
Desirable
  • Non-Medical Prescribing qualification
Experience Essential
  • Experience in older adult care
  • Excellent communication and organisational skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Attention to detail and a commitment to patient-centred care
  • A full UK driving licence and access to a vehicle (essential, due to travel between sites)
Desirable
  • Experience of working in primary care
  • Experience of working in a GP practice
