- Create and implement hospice plan of care for each patient.
- Ability to provide skilled nursing assessment and intervention to terminally patients.
- Provide continuous care for symptoms management or imminent death.
- Provide emotional/therapeutic support to families during difficult times.
- Delegate appropriate tasks as needed.
- Periodically "On-Call" for needs of the patients.
- Educating family and caregivers on plan of care, medication regimen, wound care, etc...
- Provide education on end-of-life care and disease progression.
- Playing a vital role in Interdisciplinary Team Meetings, creating goals and assisting patients with meeting them.
- Accurately and timely complete all documentation according to policies and procedures.
- Participate in policy and procedure reviews.
- Valid Oklahoma Nursing License (RN)
- Reliable vehicle with personal auto insurance and a valid driver's license.
- Live within service area.
- 5 years’ experience as Hospice Case Manager.
- Shares in providing 24-hour, seven days a week coverage to patients and families.
- Can successfully pass background checks.
- Participates in performance improvement program.
- Performs other duties as assigned.
- Supervises LPNs and paraprofessionals providing services to the patient with or without the LPN according to present at least every 30 days.
Experience:
This employee must be a graduate of accredited school of Nursing, Current RN nursing license in good standing with the State of Oklahoma, BLS/CPR certification, and a minimum of 1 year of experience in med-surg, geriatric or long-term care nursing. Prefer a minimum of 5 years’ supervisory experience.
ComputerSkills:
AOD software experience preferred along with basic computer skills.
Physical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
- Able to sit and or stand for up to two (2) hours per day.
- Walk three (3) to four (4) hours per day and do so rapidly when necessary.
- Assist with transfer, lifting, and pushing a wheelchair.
- Push/pull with up to 100 pounds of force.
- Frequent lifting & moving 100lbs. Must have the ability to lift, move, & transfer 100 lbs with the physical capability to perform all job-related duties.
- Climb stairs quickly.
- Must be able to possibly work weekends & holidays.
- Use Arjo, sit to stand/Hoyer lift when appropriate to assist residents from the floor.
- Must be able to occasionally balance, climb, crawl, crouch, kneel, stand, stoop, bend, and twist at the waist, reach above or below shoulder heights, frequently grasp and handle paperwork and diagnostic tools.
- Daily use of computer, telephone, and other diagnostic equipment.
