FULL-TIME REGISTERED NURSE CASE MANAGER position for Yukon Home Health Office.
Responsibilities- Supervise and manage all care given to assigned case management patients.
- Oversee nursing staff that are assigned to patients as ordered by physician.
- Coordinate work schedule to fit patient's time and needs.
- Complete patient visit documentation and submit electronically using Well Sky (Kinnser) EMR system.
- Current Registered Nursing License approved by the Oklahoma Nursing Board of Regents.
- At least 1 year experience in either home health setting or acute-care setting with experience in elderly, middle-aged and/or pediatric care.
- Able to lift a minimum of 20 lbs without difficulty.
- Must own a vehicle in which to make visits and maintain current automobile insurance coverage.
- Must have a telephone available for after-hours calls when needed.
