As an ACT Substance Abuse Specialist, you will provide and coordinate substance abuse interventions for our most vulnerable members. You will also serve as the substance abuse expert for an ACT team, helping the team identify and address co-occurring disorders and appropriate interventions. You will provide individual and group substance use education and interventions, as well as teaching activities and services to help the members we serve build their skills and enhance their abilities to live independently and effectively in the community.

You will provide direct services to members and develop plans to help them reach their goals, and review each member’s progress daily. You will work with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, other health care providers and involved parties to ensure that all services prescribed in each member’s individualized service plan are implemented.
