Mental Health, Substance Abuse Counselor
As an ACT Substance Abuse Specialist, you will provide and coordinate substance abuse interventions for our most vulnerable members. You will also serve as the substance abuse expert for an ACT team, helping the team identify and address co-occurring disorders and appropriate interventions. You will provide individual and group substance use education and interventions, as well as teaching activities and services to help the members we serve build their skills and enhance their abilities to live independently and effectively in the community.
The ACT model is a service delivery model, not a case management program. You will provide direct services to members and develop plans to help them reach their goals, and review each member’s progress daily. You will work with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, other health care providers and involved parties to ensure that all services prescribed in each member’s individualized service plan are implemented.
You must be passionate about pursuing best practice outcomes for members in all areas of their lives, including continuing education, employment, community tenure, and independent housing.
A complete job description is available upon request.
