RN / LPN / Nurse – RN Transfer Coordinator | Patient Care | Nights

• Provides direction, guidance, and coordination to healthcare providers for optimal, safe, effective and timely patient placements and movement; manages in-coming acute care transfers, inpatient outgoing acute care transfers and direct admissions.
• Communicates with referring hospital to coordinate transportation arrangements and bed assignment; assists with screening and facilitating unscheduled/urgent admissions.
• Connects sending and accepting physicians by physician-to-physician call.
• Interacts with physicians, nursing staff and EVS to effectively communicate patient bed placements and coordinate patient throughput.
• Assists case management with outgoing acute transfers from the emergency department as needed.
• Ensures safe and timely placement of patients based on admission status, physician-ordered level of care, patient clinical information, and unit criteria.
• Maintains documentation of activities in appropriate logs and tracks barriers to patient intake flow.
• Exhibits and models exceptional customer relationship behaviors, including telephone and in-person contact.
• Other duties as assigned.

• Essential:
  Basic Life Support (BLS)
• Essential:
  Registered Nurse (RN)
• Education:
  
  Associate's Degree
• Licensure:
  Registered Nurse with Arizona BLS (AHA)
• Three+ years of RN experience;
  Case Management experience preferred
