Summary

The Case Manager II supports the physician and interdisciplinary team in the provision of patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers. This role integrates and coordinates utilization management, care facilitation, and discharge planning functions.

Responsibilities
  • Coordinate and facilitate patient care progression throughout the continuum, working collaboratively and maintaining active communication with physicians, nursing, and other multidisciplinary team members to effect timely, appropriate patient management.
  • Ensure appropriate clinical pathway assignments by staff nurses; address and resolve system problems impeding diagnostics, treatment progress, and obstacles to discharge; seek consultation from appropriate disciplines to expedite care.
  • Utilize conflict resolution skills as necessary to ensure timely resolution of issues; collaborate with the physician and all members of the multidisciplinary team to facilitate care for the designated caseload and monitor patient progress, intervening as necessary to keep the plan of care patient‑focused, high quality, efficient and cost effective.
  • Complete utilization management and quality screening for assigned patients; apply approved utilization acuity criteria, identify at‑risk populations, monitor length of stay and ancillary resource use, and take actions to achieve continuous improvement.
  • Refer cases and issues to physician advisor in compliance with department procedures and follow up as indicated.
  • Manage all aspects of discharge planning for assigned patients: assess needs, develop an individualized continuing care plan, collaborate with external case managers, maintain plan consensus, and facilitate referrals through the resource center for home health care, hospice and medical equipment.
  • Participate actively in clinical performance improvement activities, assist in the collection and reporting of financial indicators, and use data to drive decision making and implement performance improvement strategies related to case management.

  • BASIC LIFE SUPPORT (BLS)
  • REGISTERED NURSE (RN)
Education
  • Associate's Degree
Minimum Education Required
  • Associate’s Degree
  • AZ RN license
  • BLS (AHA)
  • 3+ years related experience
Preferred
  • BSN
  • Commission for Case Manager Certification (CCME)
  • 5+ years related experience
