SummaryThe Case Manager II supports physicians and the interdisciplinary team in delivering patient care, with the goal of improving clinical outcomes and patient satisfaction while managing cost and providing accurate information to payers. This role coordinates utilization management, care facilitation, and discharge planning.
Responsibilities- Coordinate patient care progression throughout the continuum and maintain active communication with physicians, nursing, and other team members to effect timely, appropriate management.
- Ensure appropriate clinical pathway assignments by staff nurses and address system issues that impede diagnostics or treatment.
- Seek consultation from appropriate disciplines to expedite care and discharge; apply conflict‑resolution skills as needed.
- Collaborate with the physician and multidisciplinary team to monitor the plan of care, ensuring it is patient‑focused, high quality, efficient, and cost effective.
- Complete and report diagnostic testing, discharge plans, modifications of place of care, and communication to third‑party payers.
- Document in TQ screens and patient records.
- Collaborate with medical, nursing, and ancillary staff to eliminate barriers to efficient care delivery.
- Complete utilization management and quality screening for assigned patients, applying approved criteria.
- Monitor length of stay (LOS) and ancillary resource use, taking actions for continuous improvement.
- Refer cases to physician advisor and follow up as indicated.
- Ensure all elements of the plan of care and clinical path are communicated and documented.
- Manage all aspects of discharge planning, including assessment, planning, implementation, teaching, and ongoing evaluation.
- Collaborate with external case managers and multidisciplinary teams throughout the discharge process.
- Initiate referrals through the resource center for home health, hospice, and medical equipment as needed.
- Participate in clinical performance‑improvement activities and report financial indicators such as LOS, cost per case, re‑admission rates, and denial statistics.
- Use data to drive decision-making and implement performance‑improvement strategies.
- Perform other duties as assigned.
- Associate’s Degree
- AZ RN license
- BLS (AHA)
- 3+ years of related experience
- BSN
- Commission for Case Manager Certification (CCME)
- 5+ years of related experience
