As a Registered Nurse in Home Health Care, you will play a crucial role in patient care. Your responsibilities include:

• Conducting patient visits and coordinating care with all involved disciplines
• Acting as the primary contact for care providers and overseeing visit frequency
• Consulting with physicians regarding patient care details
• Performing accurate OASIS collections and maintaining updated medication profiles
• Ensuring timely reporting of lab values to physicians
• Participating in weekly case conferences and monthly case manager meetings

To succeed in this role, you must meet the following requirements:

• Possess a current Registered Nurse (RN) license in your state of employment
• Have a minimum of one year of clinical experience (preferred)
• Exhibit strong knowledge and skill in nursing practices
• Hold a valid state driver's license and auto liability insurance
