As a Registered Nurse in Home Health Care, you will play a crucial role in patient care. Your responsibilities include:
- Conducting patient visits and coordinating care with all involved disciplines
- Acting as the primary contact for care providers and overseeing visit frequency
- Consulting with physicians regarding patient care details
- Performing accurate OASIS collections and maintaining updated medication profiles
- Ensuring timely reporting of lab values to physicians
- Participating in weekly case conferences and monthly case manager meetings
To succeed in this role, you must meet the following requirements:
- Possess a current Registered Nurse (RN) license in your state of employment
- Have a minimum of one year of clinical experience (preferred)
- Exhibit strong knowledge and skill in nursing practices
- Hold a valid state driver's license and auto liability insurance
