Responsibilities- Coordinate nursing care for inpatient units, including direct patient care, patient/family education, and transitions of care.
- Support professional nursing practice across practice settings and the continuum of care; adhere to the Clinic Nursing Professional Practice Model.
- Provide leadership through roles such as preceptor, informal/formal leadership, and quality improvement efforts.
- Delegate patient care according to skill level, patient acuity, resources, and fiscal considerations.
- Demonstrate excellent communication, mentoring, and teaching abilities; participate on committees or projects.
- Operate within the Scope and Standards of Practice as outlined by the American Nurses Association and state law.
- Inspire hope and nurture the whole person; listen attentively to patients, visitors, and colleagues; deliver personalized service.
- Maintain a safe environment through adherence to 5 safe behaviors (pay attention to detail, communicate clearly, question/receptive attitude, hand off effectively, support each other).
- Collect and analyze assessment data; interpret data to determine diagnoses/issues; use evidence-based assessment techniques; validate with patient, family, and healthcare team.
- Prioritize data based on patient condition; identify risks or barriers; document in accordance with guidelines.
- Develop and implement care plans with measurable outcomes; involve patient, family, and healthcare team; adjust plans as needed; document outcomes using standardized terminology.
- Deliver and coordinate care in partnership with patient, family, and healthcare team; use evidence-based interventions; tailor interventions to patient needs.
- Prioritize interventions and perform treatments per procedural guidelines; document care in the health record; communicate via hand-offs across care levels.
- Promote health and safety; utilize teaching strategies; support self-management and health maintenance; maintain a clean, organized environment.
- Evaluate progress toward outcomes; reassess and revise diagnoses, outcomes, and plans; involve patient and family in evaluation.
- Uphold professional ethics; maintain a therapeutic nurse-patient relationship; advocate for patients and families; encourage safety and quality improvement.
- Participate in ethical decision-making and utilize available resources to address concerns; document promptly.
- Integrate evidence-based practice and research findings; participate in evidence-based activities appropriate to the role; share findings with the team.
