The Director of Case Management is responsible for leading the hospital's case management department and ensuring effective utilization review, discharge planning, interdisciplinary care coordination, and safe patient transitions.

The Director oversees a diverse team consisting of RNs, social work professionals, an LPN, and other healthcare professionals. Current case manager assignments average approximately 13 patients per case manager.

The Director generally does not carry a regular patient assignment, although they are expected to understand the team's patient population, remain engaged in difficult cases, assist during periods of low census or staffing constraints, and actively support safe and effective discharge planning.

• Provide operational and clinical leadership for an 11-person case management department
• Oversee hospital-based case management, utilization review, and discharge planning activities
• Guide the team in developing safe, timely, and appropriate patient discharge plans
• Maintain awareness of active patient cases and provide direction on complex discharge situations
• Partner closely with physicians, nursing, therapy, social work, hospital leadership, patients, and families
• Coach and develop case management staff while maintaining department performance standards
• Support regulatory compliance, documentation standards, utilization management, and performance improvement initiatives
• Participate in interdisciplinary discussions regarding patient progression and barriers to discharge

• Bachelor of Science in Nursing required for RN candidates
• Active Registered Nurse license required for nursing candidates
• Minimum 3 years of hospital-based Case Management experience
• Demonstrated experience with both Utilization Review and Discharge Planning
• Previous leadership, management, supervisory, or significant team-lead experience strongly preferred
• Proven ability to communicate effectively across interdisciplinary hospital teams
• Strong understanding of safe discharge planning and hospital case management workflows
• CCM or ACM certification preferred but may be obtained within the first year of employment

We are representing an established inpatient rehabilitation hospital in Yuma, Arizona seeking an experienced Director of Case Management to lead its hospital-based case management function.

This is a highly visible leadership position overseeing an interdisciplinary team of 11 professionals and working closely with hospital leadership, physicians, nursing, therapy, patients, families, and external care partners.

The hospital currently operates 61 beds with an average daily census of approximately 56 patients and has a planned 20-bed expansion expected to open in 2027, increasing total capacity to 81 beds.

The organization is particularly interested in an experienced RN case management leader who understands both the clinical and operational sides of hospital case management.
