As a Nurse Practitioner specializing in in-home palliative and hospice care, you’ll have the flexibility to manage your schedule while delivering personalized, dignified care to patients and families during life’s most important moments.
- Conduct in-home visits, physical assessments, and face-to-face evaluations
- Develop and manage individualized care plans in collaboration with physicians and care teams
- Prescribe medications and treatments, ensuring symptom control and comfort
- Educate and support patients and families through end-of-life care decisions
- Serve as a trusted resource for hospice eligibility and recertification

Work with an interdisciplinary team that shares your commitment to excellence
- Master’s degree and completion of an accredited Nurse Practitioner program
- Current state licensure and CPR certification
- Certification in Hospice and Palliative Nursing (CHPN) or specialty areas such as geriatrics or pediatrics preferred, but not required
- At least 1 year of NP experience and 1 year in hospice or Palliative care
- Strong communication, problem-solving, and clinical skills
- Valid driver’s license, auto insurance, and reliable transportation
