LPN Peds Private Duty Day Shift - Zachary

Position Overview

As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care to acutely or chronically ill patients in their homes, record clinical notes, and coordinate other care activities for patients in the geographic service area. You will work under the direct supervision of a Registered Nurse or designated supervisor.

• Provide high quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Document patient visits thoroughly in the electronic medical record, using the agency's visit verification program as required.
• Participate in care coordination, reporting patient needs and changes, attending case conferences, communicating with patients, families, physicians, clinicians, agency staff, case coordinators, and community case managers.

• Current and unrestricted Licensed Practical Nurse licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, or access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client characteristics.

• Home care experience.
• Ability to work flexible hours as needed.
• Ability to manage multiple tasks simultaneously.
• Ability to work independently.
• Strong communication, writing, and organizational skills.
