Position Overview

Care Management Consultant – Cardiovascular Surgery Department

Work Shift:

Day Shift (United States of America) | Scheduled Weekly

The Care Management Consultant operates within the full scope of nursing practice, serving as a patient advocate and navigator. The role develops longitudinal personalized care plans, coordinates interdisciplinary provider efforts, and leverages data analytics to manage population health.

Essential Duties
  • Transitional and Longitudinal Care Planning and Execution
    • Conduct assessments of patient/family needs and formulate documented plans to ensure continuity of care for high‑risk patients.
    • Coordinate care continuity with patients and families following admission, discharge, and ER visits.
    • Delegate tasks to assistive personnel, holding them accountable for completion.
    • Collaborate with physicians, hospitalists, PCPs, APPs, and specialists to coordinate interdisciplinary activities for high‑risk clinical and benefit decisions.
  • Communication and Coordination Between Care Settings
    • Identify appropriate providers and facilities across the continuum and maintain positive relationships.
    • Serve as coordinator and manager for defined health populations across inpatient, outpatient, and community settings.
    • Collaborate with primary care practices to individualize patient assistance and improve care quality.
    • Guide development of multiple health‑care partnerships to achieve positive health outcomes.
    • Ensure transitions of care are completed and patients have resources to improve outcomes and prevent readmissions.
    • Educate patients on system‑wide care management and ensure availability for questions and concerns.
  • Utilization Management and Data Analytics
    • Manage high‑risk patient care, including those with multiple comorbidities or high readmission risk, using patient registries when appropriate.
    • Participate in data collection, outcome reporting, clinical audits, and programmatic evaluations.
    • Analyze data to identify utilization patterns, promote cost reduction, and enhance quality of care.
    • Assist in ongoing root‑cause analysis and action‑plan development for readmissions.
Required Skills
  • Excellent oral and written communication, customer service, leadership, and teamwork skills.
  • Ability to work independently, set priorities, and coordinate care efficiently.
  • Highly organized, detail‑oriented, and capable of handling multiple tasks.
  • Proficient in motivational interviewing, teach‑back method, and self‑management support.
  • Sound judgment, decision‑making, and problem‑solving abilities.
Qualifications
  • Level 1 – Current Ohio RN, ADN, and ≥3 years clinical RN experience; or Licensed Social Worker (LSW) or Counselor (LPC) in Ohio with a bachelor’s degree in Social Work.
  • Level 2 – Current Ohio RN, ADN, 3 years clinical RN experience, 2+ years in CM/UR/Navigator/Coordinator/leadership roles or equivalent certification; demonstrated leadership and project‑management skills.
  • Level 3 – Current Ohio RN, ADN, 3+ years clinical RN experience, 5+ years in CM/UR/Navigator/Coordinator/leadership roles or equivalent certification; demonstrated advanced leadership, project‑management, KPI creation, and goal‑setting skills.

  • Level I:
    Committee participation, performance‑improvement input.
  • Level II:
    Lead performance‑improvement initiatives, mentorship, and training.
  • Level III:
    Lead projects, provide case study information, and offer advanced leadership.

  • Maintain a positive environment and uphold patient privacy and confidentiality.
