Registered Nurse; RN Zanesville, Ohio

We are looking for a Full-time or Part time RN to join our growing home care services who is experienced in nursing, with emphasis on community health education/experience to meet the needs of individuals and families within their home and communities. The position is flexible and you will be able to control your schedule.

• Complete initial home health assessment of patient and family to determine home care needs.
• Conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions.
• Supervisory visits.
• Supervising LPN and Home Health Aides.
• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Counsel the patient and family in meeting nursing and related needs.
• Provide health care instructions to the patient as appropriate per assessment and plan of care.
• Prepare clinical notes and updates.
• Clinical Record Review.
• Take call in a call rotation.

• Must have 1 year of nursing experience, reliable transportation.
