Schedule:
Monday, Tuesday, Saturday 8:00‑5:00 pm. Two holidays per year.
Territory:
Zanesville – Roseville, Thornville, New Concord, Duncan Falls, Chesterhill, Stockport, Rushville, New Lexington areas.
As a Registered Nurse (RN), you will provide one‑on‑one patient care in patients’ homes, working at the top of your license. You will employ leadership skills to coordinate care and deliver home‑based nursing services directed by an attending physician. You will be part of an interdisciplinary team that focuses on compassionate, high‑quality care and positive outcomes, interacting with patients’ families throughout their recovery journey.

How You'll WorkYou will work autonomously, managing your time efficiently and performing all practices and duties within your scope of practice as outlined by state regulations. You will administer medication, participate in interdisciplinary care coordination, and maintain comprehensive clinical documentation in accordance with agency protocol and federal guidelines.
Major Areas of Responsibility- Observe and monitor patient conditions, perform OASIS assessments to develop individualized care plans and adjust as needed.
- Administer medications as prescribed and prioritize visits for high‑risk patients to reduce rehospitalizations.
- Promote continuity of care by coordinating admissions, transfers, and discharges.
- Educate patients and families on disease processes using teach‑back methods.
- Complete all clinical documentation and collaborate with interdisciplinary team and physicians to ensure optimal care.
- Adhere to agency policies, procedures, rules, and regulations and communicate schedule changes to schedulers or supervisors.
- Attend in‑service training and mandatory agency meetings.
- Compassionate, positive communicator.
- Patience and ability to work with patients, families, physicians, and coworkers.
- Attention to detail and observance of instructions.
- Registered Nurse with current state license.
- Minimum one year of acute‑care RN experience; home‑care experience preferred.
- Valid driver’s license and auto insurance.
- Capable of meeting all physical demands of the role.
