Heartland Hospice seeks an RN Case Manager to lead personalized hospice and palliative care in patients' homes. You will be the primary nursing contact, develop and document plans of care, and coordinate with an interdisciplinary team to ensure compassionate, high-quality services.
This full-time, dayshift position requires a current RN license, at least one year in hospice or home health preferred, strong assessment and communication skills, and commitment to patient- and family-centered care.
