Monday-Friday | 8:00 AM-5:00 PM (with on call requirements)
$7,500 Sign On Bonus

What You'll Do- Provide skilled nursing care according to physician orders and RN-developed plans of care
- Assess and monitor patient conditions and communicate changes to the RNCM
- Administer medications and treatments as prescribed
- Educate patients and families on disease management, medications, safety, and preventative care
- Involve patients and families in their plan of care
- Complete accurate and timely clinical documentation in accordance with agency and Medicare requirements
- Communicate with the care team and scheduler regarding patient needs and schedule changes
- Participate in on-call coverage, case conferences, in-service training, and required meetings
- Provide care within the LPN scope of practice and applicable state and federal regulations
- Travel to patients' homes within the assigned geographic area
- Current, active LPN license in the state of Ohio
- Minimum of 1 year of acute care LPN experience required; 2 years preferred
- Home health experience preferred
- Strong communication and interpersonal skills
- Compassionate, patient-focused approach
- Ability to work independently, prioritize responsibilities, and make appropriate nursing judgments
- Strong attention to detail and reliable work ethic
- Ability to work effectively as part of a team
- Valid driver's license and auto insurance with your name listed as a driver
- Graduate of an approved practical nursing program
- Current, active LPN license in the state of Ohio
