Care Coordinator
Muskingum County, OH.
Apply for a position that offers a sign‑on bonus of $3,000. Pay range: $20.19 - $25.03 per hour based on experience, education, and/or licensure.
Integrated Services for Behavioral Health (ISBH) is a community‑mindful behavioral health organization that serves families across southeastern and central Ohio. We connect clients to comprehensive care and community resources.
Essential Functions- Joins with family to identify care coordination needs/services in line with service delivery standards and program outcomes to ensure best outcomes.
- Works with families to define cultural factors that influence strengths, functioning, and family interaction styles for ongoing engagement and success in care planning.
- Identifies strengths of children, youth, and families and uses them in care coordination engagement to support healthy outcomes.
- Coordinates family‑based services for children, youth, and families in home, school, and community.
- Ensures that services identified on care plans are appropriate, least restrictive, and meet safety and treatment needs of the child, youth, and family.
- Engages and builds positive relationships with children, youth, and families in coordination with child and family teams to support integration of team members and care plans.
- Develops collaborative and creative partnerships with community resources to meet diverse needs of youth and families.
- Maintains necessary documentation, participates in program evaluation, attends team and program planning meetings, cross‑systems training and acquires knowledge of community resources.
- Remains current with all training requirements, including High Fidelity Wraparound, MI, Cultural Humility, etc.
- All other duties as assigned.
- Provides experience in supporting children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services.
- Three years of experience with a high school diploma or equivalent; or two years with an associate degree or bachelor’s degree; or one year with a master’s degree or higher.
- Knowledge and experience in Hi‑Fidelity Wraparound preferred (Certification provided at time of employment).
- Two years of experience in a coordinated supportive services or care coordination role preferred.
- Experience working with people with autism spectrum disorders and developmental disabilities preferred.
- Experience in at least one of the following areas:
- family systems
- community systems and resources
- case management
- child and family counseling or therapy
- child protection
- child development
- Demonstrates cultural humility or responsiveness, with training and experience to manage complex cases.
- Has the qualifications and experience to work with children and families experiencing serious emotional disturbance, trauma, co‑occurring behavioral health disorders, and who are engaged with one or more child‑serving systems.
- Excellent organizational skills with ability to prioritize multiple tasks.
- High degree of cultural awareness.
- Experience with multi‑need individuals and families.
- Broad knowledge of community service systems.
- Willing to participate in and lead cross‑systems care coordination.
- Effective verbal and written communication skills.
- Proficient in operating within an internet‑based, automated office environment.
- Valid driver’s license.
- Ability to conduct video calls/meetings in an automated office environment.
- Requires minimum 100 Mbps high‑speed internet connection.
