Position Summary

The MDS Coordinator is essential to ensuring accurate, timely completion of all Medicare and Medicaid case-mix documentation. This role directly supports proper reimbursement while helping maintain the highest standards of resident care.

You will lead and oversee the full MDS process, including assessments, care planning, interdisciplinary collaboration, and regulatory compliance.

Key Responsibilities
  • Complete and oversee all MDS assessments (PPS, quarterly, annual, and significant change)
  • Coordinate interdisciplinary team involvement in the MDS process
  • Ensure accuracy, timeliness, and compliance with all regulations
  • Maintain MDS schedules and tracking systems
  • Submit and validate MDS assessments electronically
  • Lead resident care conferences
  • Collaborate with clinical teams to develop and update care plans
  • Monitor outcomes and adjust care plans as needed
  • Manage level-of-care changes and communicate updates across departments
  • Ensure timely submission of required documentation to state agencies
Qualifications
  • Current, unencumbered RN license
  • Long-term care experience preferred
  • Strong organizational and communication skills
  • Ability to collaborate effectively across departments
Physical Requirements
  • Ability to sit, stand, walk, and bend regularly
  • Ability to lift and/or move up to 25 pounds
  • Adequate visual and auditory skills for effective communication
  • Comfortable working in a moderate-noise environment
