Home Health Registered Nurse (RN) Responsibilities- Providing services in accordance with the plan of care
- Making the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisions
- Monitoring and evaluating patients' progress and updating care plans accordingly
- Educating patients and their families on managing the conditions and promoting wellness
- Informing physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible care
- Taking on‑call rotation for weekends, and holidays as assigned
- Participating in in‑service programs, supervising and teaching other nursing personnel as necessary
- Valid Registered Nurse (RN) license in the state of Illinois
- Minimum of one year of nursing experience, preferably in home health or a related setting
- Proficient in using electronic medical record systems, such as Axxess
- Previous experience in geriatrics and home health
- Ability to work independently and prioritize tasks effectively
- Ability to provide care with empathy, respect, and understanding
- Strong communication and interpersonal skills
