Position: PRN LPN for Lake, McHenry, Cook, and DuPage Counties
Home Health LPN Responsibilities:

Serving as the patient’s advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care Taking and monitoring patient vital signs such as blood pressure, temperature, and pulse

Administering medication and following healthcare plans developed by the patient's interdisciplinary healthcare team Reporting changes in patient behavior or health status to other nurses, physicians, therapists, by updating the EMR in a timely fashion

Providing education and support for patients and their family members

Required skills and qualifications for the Home Health LPN:

Graduate of an accredited school of professional nursing

Licensed as an LPN in the state of ILOne year related experience, including recent Medicare home health experience preferred

Must have a reliable car w/current insurance coverage and a current driver’s license

Experience with Electronic Medical Records (EMR) devices preferred but not required
