• Provide comprehensive skilled nursing visits in patients' homes
• Perform patient assessments and develop individualized nursing care plans
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, and Discharge visits
• Accurately complete OASIS assessments and clinical documentation within required time frames
• Educate patients and caregivers on disease management, medications, treatments, and safety
• Coordinate patient care with physicians and the interdisciplinary care team
• Monitor patient progress and promptly report changes in condition
• Ensure compliance with Medicare, CMS, ACHC, and agency policies and procedures
• Maintain accurate, timely, and complete electronic documentation
• Participate in case conferences and other meetings as needed

• Active Illinois Registered Nurse (RN) license
• Minimum of one (1) year of home health nursing experience
• Knowledge of OASIS documentation
• Knowledge of Medicare home health regulations
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong assessment, communication, and time-management skills

• Experience using Axxess EMR
• Previous Medicare-certified home health experience

Patient homes throughout the agency's service area in Illinois. More commonly in Lake, Cook, McHenry, Kane, and DuPage counties.
