Oregon Supplier Renewal

Position: Oregon Supplier Renewal 2025-2026
The purpose of this requisition is to obtain documentation for the State of Oregon  Renewal.

Please follow the instructions below:

Download and read all of the attachments. Use the COI Guides when completing your Certificates of Insurance. Read the Policies and Procedures as changes may have occurred since your previous renewal.

Attach required documents to the corresponding deliverable in the Phase/Milestone Deliverable table.

Submit the completed proposal before the Submission Deadline.

If you have questions, please leave a comment in the 'Proposal Comments' section.

It is mandatory to submit the required documentation within the system via a proposal. Attach the required documents to the appropriate deliverable.

Add documentation for each marketplace you would like to continue working in. This requisition is accepting documents for the following marketplace:

The Certificate of Existence is obtained from the Secretary of State's Office in which you are licensed to do business.

Please provide an actual certificate; A screenshot from the State's website will not be accepted.

Attach the official Certificate of Existence to deliverable #2.
07/01/2025 06/30/2026 3 Info Cubic Commercial Credit Check
Please use the link in the description and click Order Business Credit Report Now.

The report must be dated within the last 6 months. Call Info Cubic at (877) 360 - 4636 if assistance is needed.

Attach the complete report to deliverable #3.

Utilize the Certificate of Insurance guide (COI guide) in the attachments section to meet the minimum requirements.

The Certificate Holder section should state:

The entire agreement must be attached, not just the signature pages. This document requires two signatures. SIGN both signature pages before uploading the document.
