• Review patient charts using various practice EMRs to extract proof of care and submit documentation through payer portals
• Identify and communicate necessary actions to address unmet clinical quality measures
• Maintain up-to-date knowledge of quality measure specifications for electronic medical records
• Collaborate with the Clinical Ambassador for outreach efforts when required
• Participate in quality-specific projects with other departments to resolve data discrepancies

• High school diploma or GED equivalent required
• Minimum of 2 years of experience in a medical office or healthcare setting
• At least 1 year of experience with HEDIS or healthcare quality measures
• Proficiency with a variety of electronic health records and comfort with technology including Microsoft Office
• Strong communication and relationship-building skills, with a proven ability to partner with physicians and care teams

• Experience with Salesforce and Athena is a plus
• Previous experience in primary care setting
