One Medical’s High Risk team is seeking a Field Care Coordinator to support patients with care outside the traditional clinic setting – in their homes and over the phone after an acute inpatient event and as part of routine primary care services. Reporting to the Tele‑Assist Program Operations Manager, Field Care Coordinators go into patients’ homes to facilitate telehealth visits with a provider, building a trusted and supportive relationship, performing clinical assessments, assessing home safety, documenting medications, and reviewing provider care plans with the patient.
You will also assist patients and family members in using technology to coordinate care with their teams, as needed.
You are compassionate, kind, and open‑minded, comfortable engaging with patients and their caregivers or family inside their homes. You possess excellent communication skills, quickly build trust and rapport, and advocate for patients with mastery of human connection and a strong drive for service. You are passionate about removing barriers to care and use innovative problem‑solving skills to tackle tasks and challenges, resulting in a truly exceptional patient experience.
You enjoy collaborating with an integrated primary care team to facilitate better health outcomes for our patients and are comfortable driving throughout Atlanta to reach patients in their homes.
- Facilitate telehealth visits in patients’ homes, using technology to ensure the provider has a complete view of the living situation and to allow the patient exam to be completed via video.
- Conduct standard patient assessments and documentation to facilitate a complete visit by the treating provider and to ensure patients understand their care plan.
- Provide best‑in‑class telehealth set‑up services and assist with extended scope duties such as clinician‑guided physical exam, health status screenings, non‑blood specimen collection, spirometry, retinopathy, EKGs, vitals.
- Coordinate care with patients, families and outside care teams (specialists, home health, etc.) as needed.
- Assist patients and families in fully participating in health‑care services and navigating community services or other resources to support their goals.
- Participate in daily operations of the High Risk Care Team, including answering phone calls, responding to emails, and facilitating supports for transitions of care services.
- Teach patients and family members to connect with their care teams via technology.
- At least 1 year of experience in home‑based health‑care services or full‑time Emergency Medical Technician (EMT) experience.
- Certified Medical Assistant (MA) and Basic Life Support (BLS) preferred, or ability to obtain certifications within 6 months of hire.
- Strong written and verbal communication skills;
Spanish speaking is a plus. - Proven ability to persist through change, demonstrate a forward‑thinking perspective under pressure, and consistently take action on challenges.
- Proven ability to build rapport and trust with patients or customers while delivering individualized, human‑centered, customer‑focused care.
- Clinical skills in performing basic vitals and clinical assessments preferred.
