Visiting Nurse Health System is looking for an experienced and mission-driven Senior Manager of Case Management - EDWP to partner with the Director of Case Management (DCM) to oversee care coordination across community-based programs. In this highly visible leadership role, you will strengthen daily operations, support team performance, and help ensure members receive coordinated, high-quality services. This in-office position is based at our Hartsfield or Marietta office, Monday through Friday.

The Senior Manager of Case Management (SMCM) for Visiting Nurse Health System’s (VNHS) Elderly and Disabled Waiver Program (EDWP) manages

Team Leadsand case management teams, ensures high-quality member support, and aligns daily operations with organizational goals, regulatory standards, and best practices in care management.

The SMCM supports interdisciplinary teamwork,monitors performance metrics, andcollaborateson strategies to improve member outcomes, access to services, and continuity of care.

The SMCM workswith the DCM to develop and execute departmental initiatives, streamline workflows, and address barriers to member care in community settings.
