Job Responsibilities
- This position evaluates the health conditions of People Living with HIV/AIDS (PLWHA), appraising their needs, and recording the history of their encounter in a detailed fashion to ensure a comprehensive account of the patient’s well-being and visit.
- Assess, plan for, and provide basic nursing skills, in addition to performing CPR and, for some sites, AED.
- Develop and implement plans of care; understand nursing policies, procedures, protocols, treatments, standards of care and Nurse Practice Act; immunization regulation and protocols; pharmacology; and procedures for medication administration (oral, sublingual, IV).
- Tailor care provision and interventions based on the needs of PLWHA and the resources available.
- Provide treatments, administer medications/vaccines and deliver preventive and maintenance care to PLWHA and families consistent with nurse protocols.
- Screens and refers PLWHA to other internal and/or external providers as appropriate.
- Assist in planning, organizing and evaluating program areas to ensure the goals and objectives of the program are met.
- Prepare indicated clinic reports as assigned.
- Apply principles of population health to practice.
- Conduct comprehensive assessments of PLWHA’s health care needs.
- Develop comprehensive plans of care for PLWHA, families, and populations.
- Modify PLWHA treatment plans as indicated by patient responses and conditions.
- Participate in PLWHA intake and orientation for program services.
- Provide health education instruction and/or classes to PLWHAs and families for the promotion and maintenance of health.
- Participate in individualized clinical, IT, and administrative trainings as assigned.
- Participate in drills and exercises on pandemic influenza, mass sheltering, mass dispensing, and other areas as indicated.
- Perform job responsibilities with minimal supervision.
- May serve as lead for various clinic processes as assigned.
- Adhere to all DKPH policies and procedures.
- Perform other duties as assigned.
Minimum Qualifications
Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification
OR
Associate's degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
Preferred Qualifications
:
Prior experience working in Public Health and with People Living with HIV/AIDS (PLWHA). Knowledge of HIV Care Continuum and experience providing care to Ryan White patients. Knowledge and usage of computers and Windows Software related programs.
