Position Summary
The Advanced Practice Provider (APP) willserveasanintegralmemberofthe
Blood&Marrow Transplant and Cellular Therapy team ,providingcomprehensivecaretopatientsundergoingautologousandallogeneicstemcelltransplantation,CART-cell therapy,andrelatedcellulartherapies.
TheAPPwillparticipateinallaspectsofpatientcareacrossthetransplantandcellulartherapycontinuum,including evaluation,treatment planning,symptom management,toxicity monitoring,survivorship care,andlong-term follow-up.
Thisroleincludesbothinpatientandoutpatientresponsibilitiesandrequiresclosecollaborationwithphysicians,nurses,pharmacists,social workers,care coordinators,andreferringproviders.
Theidealcandidatewillhaveexperiencein Bloodand Marrow Transplantation ,Hematology/Oncology,Cellular Therapy,orarelatedspecialtyandpossessastrongcommitmenttoprovidinghigh-qualitycareforpatientswithcomplexhematologicmalignancies.
Clinical Management
- Conductcomprehensivepatientassessments,including histories,physical examinations,treatment evaluations,andfollow-upvisits.
- Evaluatepatientsforautologousandallogeneicstemcelltransplantationandcellulartherapyeligibility.
- Participateintreatmentplanningandongoingmanagementoftransplantandcellulartherapypatients.
- Order,interpret,andmanagelaboratorystudies,imaging,anddiagnostictesting.
- Prescribe medications,supportivecaretherapies,and treatment-relatedinterventionsinaccordancewithstateregulationsandorganizationalpolicies.
- Monitortreatmentresponseandadjustcareplansincollaborationwithattendingphysicians.
Blood&Marrow Transplant Care
- Managepatientsthroughoutthetransplantcontinuum,including:
- Pre-transplantevaluationandpreparation
- Conditioningtherapymonitoring
- Stemcellinfusion
- Engraftmentmonitoring
- Post-transplantrecoveryandsurvivorship
- Evaluateandmanagetransplant-relatedcomplicationsincluding:
- Graft-versus-host disease(GVHD)
- Infectious complications
- Organ toxicities
- Participateinbothinpatientroundingandoutpatienttransplantfollow-upclinics.
Cellular
TherapyandCART-Cell Management
- ParticipateinevaluationandmanagementofpatientsreceivingCART-celltherapyandothercellulartherapies.
- Coordinatecarethroughoutthecellulartherapytreatmentprocess,including:
- Initial consultation
- Bridgingtherapymonitoring
- Lymphodepletingchemotherapy
- Cellulartherapyinfusion
- Post-treatment monitoring
- Assessandmanagecellulartherapy-relatedtoxicitiesincluding:
- Cytokine Release Syndrome (CRS)
- Immune Effector Cell -Associated Neurotoxicity Syndrome (ICANS)
- Infectious complications
- Collaboratewithcriticalcare,neurology,infectious disease,andotherspecialtyserviceswhenescalationofcareisrequired.
Care Coordination
- Serveasaprimaryclinicalresourcefortransplantandcellulartherapypatientsandcaregivers.
- Coordinatemultidisciplinarycareamongphysicians,nurses,pharmacists,social workers,andreferringproviders.
- Facilitatetransitionsbetweeninpatientandoutpatientsettings.
- Participateindailyrounds,multidisciplinary conferences,andtreatmentplanningdiscussions.
- Ensuretimelycommunicationwithpatients,caregivers,andreferringphysicians.
- Educatepatientsandfamiliesregarding:
- Bloodandmarrowtransplantation
- CellulartherapyandCART-celltreatmentprocesses
- Potentialcomplicationsandsideeffects
- Medication management
- Infection prevention
- Long-term follow-upandsurvivorshipcare
- Reinforcetreatmentadherenceandpromotepatientengagementthroughoutthecarecontinuum.
