We are currently seeking an experienced Field Nurse Case Manager to support the Atlanta, GA area.

This is a field /
home‑based field position with travel (up to 2 hours one‑way
) to physicians’ offices, hospitals, and other care locations.
As a Nurse Case Manager at Paradigm, you will be responsible for the medical case management of work‑related injuries, including:
- Comprehensive assessment, planning, coordination, implementation, and evaluation of care
- Collaboration with injured workers, insurance carriers, medical providers, employers, and attorneys
- Monitoring clinical progress and outcomes and reporting to stakeholders
- Managing complex and catastrophic cases aligned with your expertise

- Current Registered Nurse (RN) license in Georgia
- Nationally recognized case management certification:
CCM, CRRN, COHN, or CDMS - Valid driver’s license with good driving history
- Strong organization and time‑management skills
- Excellent written, verbal, and interpersonal communication
- Comfort working independently with minimal supervision
- Proficiency with technology and documentation systems
