H2 Homecare is searching for nurses who are dedicated to providing care for patients located throughout Georgia. We support our nurses through on‑site training with our clinical supervisors, as well as a supportive team that will work with you to provide flexible hours, a supportive orientation, and training on our EMR (electronic medical record) system.

Credentials- Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the State of Georgia
- Graduate of an accredited practical nurse or vocational nursing program
- One year of prior professional nursing experience, homecare experience preferred
- Current CPR certification
- Clean background check
- Identify and document changes in the patient’s condition with resolution on the appropriate clinical form
- Notify physician to obtain necessary orders for intervention(s) per state regulations
- Perform specific treatments and medication administration in accordance with physician orders
- Ensure documentation reflects patient/family collaboration and participation in their plan of care
- Document and clock in and out using the current patient health record platform, Alayacare
- Communicate skilled observations and significant changes in patient status/care to the Director of Clinical Services and/or Clinical Supervisor/Case Manager
- Request modification of orders and additional orders based on the patient’s clinical status and skilled nursing judgment
- Document all physician collaboration/coordinating of care in the medical record and communicate as needed
- Adhere to H-squared Homecare policies and procedures
