Job DescriptionCoordinates and handles consult calls and transfers based on Transfer Center guidelines and preferences. Assesses, determines, and coordinates placement of admissions (elective and emergent) and transfers in accordance with clinical standards and guidelines. Ensures patients are placed in clinically appropriate levels of care via established preferences and protocols. Upholds systemwide customer service program standards.
Responsibilities- Coordinate patient placement activities via Transfer Center to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff.
- Maintain assigned databases.
- Collaborate in collection of data for identification and resolution of flow issues.
- Collect, analyze, and interpret outcome data systematically to identify problem areas and assess the need for improvement or support changes in systemwide processes in relation to the Transfer Center.
- Prepare periodic reports as directed.
- Participate actively and contribute to organizational initiatives that impact patient flow.
- Support continuous performance and quality improvement.
- Communicate openly with appropriate leadership team to ensure knowledge of key decision factors.
- Serve as clinical resource for patient placement by ensuring smooth functioning of Transfer Center processes to reduce wait times and avoid unnecessary patient movements.
- Maintain positive working relationships and demonstrate excellent customer‑services skills in all interactions between the Transfer Center and physicians, internal customers, and external customers.
- Assist with orientation and continuing education of peers.
- Minimum 2 years of bedside nursing experience; pediatric experience preferred.
- Bachelor of Science in Nursing from an accredited school.
- Experience with Epic, Microsoft Teams, Cisco call systems, and Red Cap preferred.
- Previous ED, NICU, and/or PICU experience.
- Licensure as a Registered Nurse in the state of Georgia or Multi‑State through the Enhanced Licensure Compact.
- Basic Life Support (BLS) certification from the American Heart Association within 30 days of employment.
- Pediatric Advanced Life Support (PALS) certification from the American Heart Association.
- Ability to manage multiple and continually changing priorities and make rapid decisions.
- Demonstrated ability to integrate pathophysiology, psychology, and sociology into the pediatric clinical setting.
- Effective verbal and written communication skills and ability to handle conflict and reach consensus and collaboration.
- Computer proficiency in a wide variety of software, including Microsoft Office applications and database management.
