We are hiring for a Home Health RN Case Manager with a Medicare-certified home health agency. This position is a full-time, direct-hire opportunity.
This opportunity is ideal for an experienced RN who thrives with clinical autonomy, values meaningful one-on-one patient relationships, and wants to practice at the full scope of nursing outside of an institutional setting. The ideal candidate is self-directed, highly organized, confident with OASIS documentation, and passionate about helping patients recover and remain safely in their homes.
Responsibilities- Conduct initial and recertification OASIS assessments to support appropriate episodes of care and reimbursement accuracy
- Develop and implement individualized care plans aligned with physician orders and CMS requirements
- Perform skilled wound care, including complex dressing changes, NPWT support, and documented wound measurements
- Administer and monitor IV therapy, PICC lines, TPN, injections, and medication-related patient responses
- Complete medication reconciliation, identify adherence concerns, and coordinate updates with physicians and pharmacy teams
- Coordinate interdisciplinary services including PT, OT, SLP, social work, and home health aide support
- Communicate patient changes, hospitalization risks, and care needs to physicians and specialists
- Monitor for clinical deterioration and coordinate timely interventions or care transitions as needed
- Complete visit notes, OASIS documentation, 485 plans, and discharge summaries according to CMS standards
- Active, unrestricted RN license in Georgia required; multi-state compact licensure preferred
- Minimum of 3 years of nursing experience required
- Minimum of 1-2 years of home health, case management, or post-acute care experience preferred
- OASIS competency required or ability to complete training within 30 days; OASIS-E experience preferred
- Experience with skilled home health procedures including wound care, IV therapy, catheter management, and point-of-care testing
- Knowledge of CMS Conditions of Participation, Medicare coverage criteria, and home health documentation requirements
- Experience with home health EMR platforms and electronic clinical documentation preferred
- Full-time, Monday through Friday
- Field-based in the Atlanta, GA metro area
- Day shift with occasional on-call rotation
The salary range for this position is approximately $70,000 – $90,000 annually ($33.65 – $43.27 per hour), based on experience and qualifications.
