Job Summary
GENERAL SUMMARY
We areseekinga highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The Care Connect Team at Well Be supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner.
This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum.
The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Urgent Care RN staff in these roles will help foster positive and effective working relations with senior leadership and management, local market management and business ancillary departments for the purpose of achieving the company's vision, mission and objectives.
S/he is responsible for following the rules and regulations of state and federal regulatory agencies and other certified agencies for their region. Daily, local travel to meet patients and the community will be an essential part of your role.
Salary
: $77,000 - $112,575 + Mileage & Full Benefits

- Deliver urgent, in-home, patient care under the direction of a licensed provider throughout the assigned market including initial assessments, routine follow-up visits, and ongoing care coordination.
- Conduct comprehensive home safety evaluations and document findings during patient visits.
- Deliver patient care under the direction of a licensed provider by:
- Obtaining patient histories
- Reviewing medications and treatment plans
- Performing physical assessments
- Identifying and addressing health concerns observed or reported during home visits
- Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures.
- Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter.
- Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team.
- Deliver high-quality, patient-centered care while meeting established clinical, quality, and operational performance standards.
- Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives.
- Complete and submit all required documentation, reports, and administrative paperwork within established timelines.
- Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning.
- Collaborate closely with APPs, nurses, social workers, behavioral health specialists, and other care team members to develop and implement individualized patient care plans.
- Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development opportunities.
- Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources to support Well Be's presence and reputation within the community.
