Job Title: Licensed Practical Nurse (LPN) – Home Care (Skilled Nursing)
Location: Atlanta, GA and surrounding areas
Employment Type: Part-Time / As needed
Pay Rate: $28 - $35 per hour (based on experience and case acuity)
Position OverviewWe are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide in-home care for clients with complex medical needs, including those diagnosed with Primary Lateral Sclerosis (PLS). This role involves delivering high-quality, patient-centered care in a home setting while working under the supervision of a Registered Nurse (RN).
Key Responsibilities- Administer skilled nursing care in accordance with physician orders and care plans
- Perform and manage PEG tube feeding and monitor tolerance
- Provide wound care and bedsore management (as applicable)
- Administer medications and monitor for side effects or changes in condition
- Monitor vital signs and report any changes to supervising RN/physician
- Assist with mobility, positioning, and prevention of pressure injuries
- Maintain accurate and timely clinical documentation
- Educate clients and families on care procedures when appropriate
- Collaborate with caregivers and interdisciplinary team members
- Active Licensed Practical Nurse (LPN) license in Georgia
- Minimum 1 year of nursing experience (home care experience preferred)
- Experience with tube feeding and wound care strongly preferred
- Current CPR/BLS certification
- Strong clinical, communication, and organizational skills
- Ability to work independently in a home care setting
- Experience with neurological conditions (e.g., ALS, PLS, stroke care)
- Comfortable working long shifts (8-12 hours)
- Compassionate, reliable, and patient-focused approach
