Job Overview

As an RN Care Manager with VillageMD, you will help our team collaborate with providers and engage high‑risk patients to develop patient‑centered care plans focused on chronic condition management and measured by clinical indicators.

Responsibilities
  • Embed in the physician office and work directly with providers to engage high‑risk patients.
  • Involve high‑risk individuals in activities to improve their health and partner with the care team to establish identified goals and action steps focused on wellness.
  • Apply clinical knowledge and a deep understanding of community resources to deliver health education and coordinate consults and referrals that support patients and facilitate continuity of care.
  • Develop relationships with primary care practice teams to operate as an extension of the provider in supporting high‑risk patients.
  • Manage a caseload of high‑risk patients, set patient‑specific goals, and provide interventions to help them better manage their health.
  • Participate in grand rounds and other meetings with provider partners and VMD leadership to discuss patient care and improve care team processes.
Qualifications
  • Registered Nurse with licensure in the state of practice.
  • Minimum of 3 years of direct, clinical nursing experience.
  • Case Management Certification is preferred, but not required.
  • Experience in care management or transitional care management that requires assessment, critical thinking, and application.
  • Comfort with technology, including Microsoft Office and a variety of electronic health records (EHR) for data capture, mining, and reporting.
  • Strong communication, time management, and organizational skills.
  • Ability to be flexible in an ambiguous and dynamic environment with a service orientation and a positive, can‑do attitude.
  • Willingness to learn independently, take initiative, and practice humility and trust‑building communication.
