Licensed Vocational Nurse (LVN) - Home Health

Assignment Duration:
This is a 13-week contract position.

Schedule:

Monday to Friday, day shifts with 5 x 8 hour shifts (40 hours per week).

Job Description:

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to provide home health care services in the Atlanta area. The LVN will be responsible for routine visits and ensuring that clients receive high-quality care tailored to their needs.

• Conduct routine visits to clients and perform necessary health assessments.
• Administer medications and treatments as prescribed.
• Document patient interactions and maintain accurate medical records.
• Collaborate with healthcare team members to formulate care plans.
• Manage caseload to meet weekly visit requirements.
• Provide education and support to patients and their families.

• Current LVN license in the state of Georgia.
• Recent home health experience is required.
• Experience with Home Care Home Base (HCHB) is preferred. Candidates without HCHB experience may be considered at a reduced rate for training.
