Responsible for assisting patients with complex heart related concerns and their caregivers through the continuum of care and supporting their decision‑making processes. Integral team member of the Cardiology Team dedicated to providing an excellent patient experience and coordinating their visit. Gathering required information from patients or external sources to enable a thorough clinical evaluation. Assembling appropriate clinical information for prospective review by the multidisciplinary team or treating physician.

Coordinating additional testing procedures as directed by the team or treating physician. Educating patients about the prescribed treatment plan. Routing patients to complementary support services as needed. Providing community education and outreach services that promote this patient centered care model.

• Graduate from an accredited school of nursing or
• Physician Assistant Program

• 2 years of nursing experience if RN Required
• A masters degree from a PA or NP program may substituted for nursing experience Required
• Experience in working with cardiac disease population [in the specialized field of cardiology care:
  Advanced Heart Failure, EP, or Structural Heart] Required

• RN
  - Registered Nurse
  - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Upon Hire Required or
• PA
  - Physician Assistant in the State of Georgia Upon Hire Required
