About the Role:

The Nurse Practitioner Full Time Palliative Care Home Visits role is dedicated to providing compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses in their homes. This position focuses on managing symptoms, improving quality of life, and supporting patients and their families through complex medical and emotional challenges. The role requires collaboration with interdisciplinary teams to develop and implement personalized care plans that align with patients' goals and values.

The Nurse Practitioner will serve as a primary healthcare provider in the home setting, ensuring continuity of care and timely interventions. Ultimately, this position aims to enhance comfort, dignity, and holistic well-being for patients receiving palliative care outside of traditional clinical environments.

Minimum Qualifications:

• Current and unrestricted Nurse Practitioner license in the United States.
• Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Nurse Practitioner (e.g., ANCC or AANP certification).
• Minimum of 2 years clinical experience, preferably in palliative, hospice, or chronic disease management.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications:

• Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
• Experience conducting home visits and working within community-based healthcare settings.
• Familiarity with electronic health record (EHR) systems and telehealth technologies.
• Strong background in pain and symptom management for complex patient populations.
• Training in cultural competency and communication skills tailored to diverse patient populations.

Responsibilities:

• Conduct comprehensive patient assessments during home visits, including physical, emotional, and psychosocial evaluations.
• Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
• Manage and monitor symptoms such as pain, nausea, fatigue, and respiratory distress using evidence-based interventions.
• Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
• Coordinate care with physicians, social workers, hospice providers, and other relevant professionals to ensure seamless service delivery.
• Document all patient interactions, care plans, and clinical outcomes accurately and timely in electronic health records.
• Respond promptly to urgent patient needs and facilitate appropriate referrals or hospital admissions when necessary.
• Advocate for patient preferences and support ethical decision-making in end-of-life care scenarios.

The Nurse Practitioner will utilize advanced clinical assessment skills daily to evaluate patient conditions and tailor care plans effectively. Strong communication skills are essential for educating patients and families, facilitating difficult conversations about prognosis and care preferences, and collaborating with interdisciplinary teams.
