Location: Atlanta, GA
Duration: 3 months contract (possibility of extension)
Shift Time: Day Shift | 08:30 AM – 05:00 PM | Every other weekend & holidays required
Local Pay Rate: $65.00 - $70.00/hr on W2 without benefits (Within 50 Miles)
Travel Pay Rate: $2,900 - $3,100/Weekly gross (Above 50 Miles)
Responsibilities Include:
- Coordinate patient care plans from admission through discharge.
- Perform comprehensive patient assessments and identify discharge needs.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to develop individualized care plans.
- Facilitate appropriate utilization of healthcare resources while maintaining quality patient outcomes.
- Coordinate referrals for home health, rehabilitation, skilled nursing facilities, and community services.
- Educate patients and families regarding treatment plans, discharge instructions, and available resources.
- Maintain accurate and timely documentation in the electronic medical record (EMR).
- Ensure compliance with hospital policies, regulatory requirements, and case management standards.
- Minimum 1 years of recent Case Management experience in an acute care hospital.
- Active Georgia Registered Nurse (RN) License or Compact State License.
- BLS (AHA)
