The Nurse Care Partner is responsible for ensuring a safe environment for patients within the hospital setting. This role involves observing and reporting any changes in the patient's condition to the nurse accountable for the patient's care under the direction of a RN. The Nurse Care Partner will communicate with patients, family members, and all members of the healthcare team to relay appropriate information, contributing to patient care and creating a supportive and healing environment.

The Nurse Care Partner's primary focus will be to promote a positive patient experience by partnering with the nurse, physician, healthcare team, and other healthcare providers to provide excellence in patient experience and assist with daily living activities and comfort measures. The Nurse Care Partner (NCP) assists other healthcare providers with safe, age-appropriate, and culturally aware care by performing clerical functions such as retrieving data, maintaining unit supplies, and transporting supplies.

Performing essential patient care functions, i.e., ADLs, ambulation, and vital signs. The Care Partner must be able to work in a fast-paced environment, which requires multi-tasking and effective communication with the Registered Nurse and ancillary team.

• Excellence in Patient Experience by partnering with healthcare team, patient and family.
• Adheres to the Fall Prevention Protocol/Restraint Management Protocol, under the supervision of the licensed professional.
• Maintains confidentiality adhering to HIPAA regulations and recommended privacy procedures.
• Proactively offers assistance and comfort measures to patients.
• Present self in positive and professional manner and show sensitivity to concerns and needs of others.
• Performs basic patient care under the direction of the Registered Professional Nurse (ADL's, ambulation, vital signs).
• Provides patient care needs (ex. Hygiene, nutrition, toiletings, physical comfort measures).
• 1:1 continuous monitoring that includes remaining within arm length or a safe distance and facing the patient at all times.
• Provides continuous supervision of selected patients who meet eligibility criteria for involuntary status under the Georgia Mental Health Code (1013, 2013, 1014, 1021 certificates) and suicidal precautions.
• Provides continuous supervision to patients adhering to the Fall Prevention Protocol/Retrain Management Protocol under the Registered Professional Nurse.
• Discard disposable supplies and assist with bed rails, bed and chair alarms, mobility, wheelchairs, walkers, canes, and bedpans under nurse supervision.
• Supports professional nursing practice through shared leadership, evidence-based practice and ethical accountability.
• Provides a safe environment for the provision of patient care.
• Consistent use of hand hygiene no artificial nails.
• Follows appropriate PPE/infection control policies.
• Keeps patient rooms clean and free of clutter.
• Utilizes proper body mechanics, patient safe assistive devices, patient safe handling devices (I.e. walkers, therapeutic safety chairs)
• Uses positive, respectful, and effective oral and written communication with all members of the healthcare team, patients, families, and visitors
  
  B. communicates patient care concerns/problems/emergencies to the Registered Professional Nurse, Real Time Communication is required.
  
  C. Uses safety communication techniques (AIDET, STAR, SBAR)
  
  D. Documents completion of patient care in EMR (Electronic Medical Record)

• High school Diploma/GED
• Two (2) or more years related including clinical experience in acute care setting, long-term care facility, or other verifiable direct clinical patient care experience. Or has enrolled in or completed an accredited nursing program or school.
• BLS from AHA or Red Cross
