Vital Living Healthcare is seeking a full‑time Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing concierge primary care and wellness practice in Atlanta, GA.
This is a unique opportunity to practice relationship‑based medicine in an environment that allows providers the time and resources to truly partner with patients in achieving their health goals. We are looking for a provider with a strong primary care foundation who is passionate about patient care, wellness, and prevention. Experience with hormone replacement therapy, weight management, or wellness medicine is a plus, but we are willing to train the right candidate.

Position OverviewThis is an excellent opportunity for a Nurse Practitioner or Physician Assistant who enjoys developing meaningful patient relationships and practicing proactive, preventive medicine. The ideal candidate will work closely with our physician and clinical team to provide concierge primary care, wellness services, hormone optimization, and medical weight management in a highly personalized setting.
Experience in hormone replacement therapy, peptide therapy, weight management, or longevity medicine is preferred but not required. We are committed to training and mentoring the right provider who brings strong primary care experience, clinical excellence, and a passion for helping patients achieve optimal health.
Responsibilities- Provide comprehensive primary care services for concierge patients, including preventive care, chronic disease management, and acute care.
- Perform patient evaluations, physical exams, and follow‑up visits.
- Diagnose and manage acute and chronic medical conditions.
- Order and interpret laboratory and diagnostic testing.
- Develop individualized treatment plans based on patient goals and clinical findings.
- Educate patients on lifestyle modifications, nutrition, exercise, supplementation, and preventive health strategies.
- Collaborate with the physician to develop wellness, hormone optimization, peptide, and weight management programs.
- Participate in hormone replacement therapy evaluations and management (training provided).
- Manage medical weight loss programs, including GLP‑1 therapies and other evidence‑based treatment options.
- Assist with peptide therapy, IV therapy, and other wellness services offered by the practice.
- Perform in‑office procedures within scope of practice, including pellet therapy (training provided if needed).
- Maintain accurate and timely documentation in the electronic medical record.
- Foster strong patient relationships through exceptional communication, accessibility, and continuity of care.
- Stay current on emerging evidence and best practices in primary care, wellness, hormone optimization, and longevity medicine.
- Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the State of Georgia.
- Minimum of 2 years of primary care experience.
- Strong clinical assessment, diagnostic, and patient management skills.
- Excellent communication and patient education skills.
- Ability to build long‑term relationships with patients.
