Now HiringWe’re seeking a dedicated MDS Coordinator (RN or LPN) to lead and manage the RAI/MDS process and ensure accuracy, compliance, and quality-driven outcomes across the center.
Position SummaryAs MDS Coordinator, you’ll play a key leadership role in driving resident-centered care by coordinating timely and accurate assessments and care planning. You'll serve as a bridge between clinical teams and administration to ensure quality data and outstanding outcomes.
Key Responsibilities- Serve as the facility's lead for all MDS/RAI processes and documentation
- Initiate and lead Interdisciplinary Care Plan meetings
- Coordinate and update resident care plans based on MDS triggers and assessments
- Collaborate with nursing staff and other disciplines to gather accurate resident data
- Ensure timely completion and transmission of MDS assessments per CMS and LTCI guidelines
- Analyze Quality Measures/Indicators (QM/QI) from MDS reports and support performance improvement initiatives
- Provide training and guidance to clinical team members as needed
- Participate actively in quality improvement programs and audits
- Must be a Licensed RN or LPN in the State of Georgia
- Experience with MDS/RAI in a skilled nursing or long-term care setting strongly preferred
- Strong understanding of CMS guidelines and compliance standards
- Detail-oriented, organized, and collaborative
- Excellent communication skills and the ability to lead interdisciplinary teams
