• Assist patients and families in understanding their diagnosis and treatment options.
• Conduct assessments, symptom management, and case management for patients with chronic diagnoses.
• Develop and facilitate individualized care plans in consultation with healthcare providers and family members.
• Serve as a link between patients, healthcare providers, and support teams.
• Facilitate changes in care plans and broker necessary services.
• Perform care assessments and triage of patients via phone or in-person.
• Facilitate appointments for consults, procedures, and support services.
• Standardize patient education materials and maintain a resource library.
• Deliver patient and family education throughout the treatment process.
• Monitor patient compliance with treatment plans and assist with referrals.

• RN License in Georgia required

• 2+ years in a nursing role preferred

• Current RN License in Georgia
• BLS certification required
