Schedule:
Monday and Wednesday - 5PM to 10PM
Advanced Care Partners is seeking a Registered Nurse (RN) to provide individualized care for medically fragile children and adults in home and school settings. This position offers a more personalized care model, allowing for stronger patient outcomes and professional satisfaction.
Responsibilities- Provide skilled nursing care (feeding tubes, tracheostomies, ventilators, central lines)
- Monitor patient progress and respond to changes in condition
- Educate patients and families on treatment plans
- Maintain a safe and clean care environment
- Active RN license in Georgia or Nurse Licensure Compact
- A minimum of one year of RN experience
- Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or equivalent
- Valid PPD skin test
Physical Requirements:
Ability to stand or sit for extended periods (up to 12 hours); ability to bend, lift, and assist with patient mobility; lift up to 50 pounds or more; fine motor skills for clinical tasks

Currently caring for patients in Port St. Lucie, with new locations coming soon to Fort Pierce and Titusville.
