POSITION TITLE: Nurse Practitioner

POSITION SUMMARY

CBWW is seeking an experienced and dependable PRN Nurse Practitioner to provide flexible clinical coverage within the Wellness Clinic.

The PRN Nurse Practitioner will provide primary, preventive, and problem-focused healthcare based on patient volume, provider availability, planned or unexpected absences, specialty clinic needs, and other operational requirements. The provider will work collaboratively with the Medical Director, Nurse Practitioners, Medical Assistants, and clinical support team to maintain safe, timely, and high-quality patient care.

Key Responsibilities

· Provide primary care, preventive care, and Problem-Focused & Same-Day Visits.

· Perform patient histories, physical examinations, assessments, diagnoses, and treatment planning.

· Manage common acute and chronic conditions, including hypertension, diabetes, and cholesterol.

· Provide STI screening, treatment, preventive care, and women’s health services when qualified.

· Prescribe medications within authorized scope of practice.

· Personally enter and manage laboratory, imaging, and diagnostic orders.

· Review and interpret laboratory and diagnostic results and ensure appropriate follow-up.

· Enter specialty referrals and coordinate additional care when clinically indicated.

· Complete accurate and timely documentation in the electronic medical record (EMR).

· Respond to patient clinical questions, results, and portal messages associated with assigned care.

· Provide appropriate clinical handoff for unresolved patient needs at the completion of PRN coverage.

· Maintain documentation necessary to support medical necessity, coding, and insurance billing.

· Collaborate with the clinical team to maintain efficient patient flow and continuity of care.

· Follow CBWW policies, HIPAA requirements, clinical protocols, and applicable professional standards.

Provider Order Responsibilty

The Nurse Practitioner is responsible for determining and entering all laboratory and diagnostic orders associated with their patient care. Medical Assistants and other unlicensed staff may not process provider-authorized orders but may not independently add, remove, substitute, or modify laboratory or diagnostic testing.

REQUIRED QUALIFICATIONS

§ High School diploma or equivalent

§ Current Georgia RN and Nurse Practitioner licensure/authorization.

§ Graduate degree from an accredited Nurse Practitioner program.

§ Current national Nurse Practitioner certification.

§ Current prescriptive authority

§ Current BLS/CPR certification.

§ Experience independenly assessing and managing patients in an outpatient clinical setting.

§ Strong clinical decision-making and patient education skills.

§ EMR proficiency and strong clinical documentation skills.

PREFERRED QUALIFICATIONS

§ Family Nurse Pratitioner (PNP) or Adult-Gerontology Primary Care Nurse Practitioner certification.

§ Two or more years of outpatient or primary care experience.

§ Experience with chronic disease management, women’s health, and STI care.

§ Experience in community health, outpatient, nonprofit, FHC, or safety-net settings.

§ Experience serving underserved and diverse patient populations.

PRN SCHEDULE

This is an as-needed position with no guaranteed minimum hours. PRN providers may provide coverage for planned and unexpected provider absences, increased patient volume, specialty clinics, telehealth, outreach events and other operational needs.
