Job Summary:
- Highly organized, tracking and follow‑up for many different job functions.
- Complete a significant number of Needs Assessments for new individuals and annual renewals for individuals on the Planning List.
- Provide telephone support to individuals and families on the Planning List, resolving phone calls and messages.
- Report Planning List changes and concerns to Planning List Supervisor.
- Make and resolve referrals to outside agencies for non‑waiver services.
- Provide other supports for Planning List as needed.
- Experience with or knowledge of individuals with intellectual/developmental disabilities, Medicaid waivers, and DBHDD I/DD services; able to work successfully without daily contact with a supervisor.
- Spanish fluency.
This position is located in the DBHDD Region 2 Field Office in Augusta, Georgia. DBHDD Region 2 serves individuals residing in the following counties:
Baldwin, Barrow, Bibb, Burke, Clarke, Columbia, Elbert, Emanuel, Glascock, Greene, Hancock, Jackson, Jasper, Jenkins, Jones, Lincoln, Madison, McDuffie, Monroe, Morgan, Oconee, Oglethorpe, Putnam, Richmond, Screven, Taliaferro, Twiggs, Walton, Warren, Wilkes, Wilkinson.

Bachelor's degree in a related field from an accredited college or university and two (2) years of job‑related experience; or two (2) years of experience at the lower level Social Services Program Coordinator 1 (SSP
030) or position equivalent.
